高齢者および在宅療養者とその家族の理解と看護援助の実践
To understand older adult patients and their families receiving home care, and to practice nursing care.

授業の目的と概要/Object and Summary of Class

地域や病院、施設で生活する高齢者とその家族、地域で生活する在宅療養者とその家族の生活と健康、および健康問題について理解し、ニーズに対応した看護支援の特徴を理解するとともに、地域包括ケアシステムにおける関係機関との連携・調整について学び、看護職としての役割を考察する。
Understand health and health problems of older adults/people received home care services and their families in communities, and how to provide nursing care according to needs. And learn collaboration and coordination of services with related health service facilities and roles of nurses in integrated community care systems.

学習の到達目標/Goal of study

１）高齢者とその家族を対象として人間関係を築き、生活と健康および健康問題について理解する。
　　To build relationships with older adults and their families, and to understand their life, health, health problems.
２）病院において健康問題を抱えている高齢者を対象に、ニーズに対応した看護援助を展開する。
To practice nursing care for older adults with health problems in hospitals according to their needs.
３）在宅療養者とその家族の生活を考慮した看護援助の実際を学び、支援の特徴について考察することができる。
To learn practice of nursing care for older adults and their families receiving home care in view of their daily living, and to consider characteristics of the home care.
４）施設で生活する高齢者の生活と健康、健康問題を把握し、支援の特徴について考察することができる。
To understand daily living, health and health problems of older adults in geriatric health service facilities, and to consider appropriate care for them.
５）病院、施設、地域における関係職種・機関との連携・調整について学び、多職種と協働するなかでの看護職の役割について考察することができる。
To learn inter-professional/ inter-facility collaboration and coordination in hospital, geriatric health service facilities and community, such as the integrated community care system, and to consider the role of nurses in the system.

授業内容・方法と進度予定/Contents and progress schedule of the class

１．訪問看護事業所/看護小規模多機能型居宅介護事業所実習
/ Practice at home care nursing agencies/community-based multifunctional long-term care and nursing facilities.
地域の訪問看護事業所/看護小規模多機能型居宅介護事業所において利用者宅へ訪問看護師と同行訪問を行う。
Visit and provide care for clients along with home health nurses of home care nursing agencies/community-based multifunctional long-term care and nursing facilities.
２．介護老人保健施設等実習/ Practice at geriatric health services facilities
特別養護老人ホームまたは介護老人保健施設で実習を行う。
Visit a welfare facility for the elderly requiring long-term care or a geriatric health services facility and communicate with older adults in the facilities.
３．病棟実習/ Practice at inpatients’ ward
急性期の治療を必要とする高齢患者を受け持ち、看護過程を展開する。
Implement a nursing process for a hospitalized older adult patient.

成績評価方法/Evaluation method

実習参加状況（実習態度や実習内容）、実習記録（レポート含む）などを総合的に判断して評価する。
Evaluation will be based comprehensively on learning attitude, perfomance in practice, nursing practice training records, and reports.
