Nursing Practice to Adult Patients with Chronic diseases and at the End of Life and Their Families
概要/Summary
成人看護学原論，成人看護方法論Ⅰで学習した看護理論・援助方法を臨地実習場面において体験し，慢性期・終末期看護の実践に必要な知識，技術，態度を統合的に修得する。
This course focuses on integrating those nursing theories and methodology for caring adults learned in previous semestar (Adult Nursing I) in clinical practice situations, and acquire the knowledge, skills, and attitudes necessary to provide practical nursing care for patinets with chronic diseases and/or at the end of life.
目的/Aim
慢性的な健康問題により生じるストレス・危機状況にある成人患者およびその家族に対し，人間関係
を基盤に問題解決の系統的アプローチを適用して，成長・発達・適応へ向けての看護を実践するための基本的な知識や技術および能力を養う。
To develop basic knowledge, skills, and abilities to practice nursing care for adult patients and their families in stressful and crisis situations caused by chronic health problems, applying a systematic problem-solving approach based on human relationships.
目標/Goal
１．健康問題に伴うストレス・危機状況にある成人患者を多面的に理解し，看護の問題解決のプロセス
の展開ができる。
To learn the process of solving health problems.
２．慢性期疾患患者が障害を受容して日常生活規則の中で自己管理と社会生活への適応ができるように
援助できる。
To help patients in chronic conditions to adapt to society.
３．近い将来，死を免れ得ない患者が意義深く生きられるような，また，尊厳を持って死を迎えられる
ような援助を理解できる。
To assist patients until death while respecting the patient's dignity.
４．患者および家族はそれぞれの価値観を持ち，自立・自律した存在であることを尊重し，人間として
の尊厳を重んじる態度がとれる。
To respond to patients and their families with respecting human dignity.
授業計画 / Contents and Progress Schedule of Class
実習スケジュールは以下に示す。
１．学内演習および見学実習（1週間）/ On-campus exercises and study by observation on ward (one week)
慢性疾患患者の援助に必要な日常生活・診療介助援助に必要な技術について演習，文献学習を行う。
各病棟実習を効果的なものにするために，緩和ケア病棟で看護師の指導の基，見学実習を行う。
Practice and literature study will be conducted on the skills necessary for assisting patients with chronic diseases in daily living and medical assistance.
In order to make the practical training in each ward effective, students observe and practice under the guidance of nurses in the palliative care ward.
２．病棟実習（2週間）/ Practice at inpatient ward (two weeks)
学生1名で，成人期にあり慢性的な健康問題を持つ患者1名を受け持ち，看護過程を展開する。
受け持ち患者を全体的に理解し，適切な問題解決の方向性を見出し援助するために，適時カンファレンスを
実施する。
One student will be assigned to one patient with chronic health problems in adulthood and will develop the nursing process.
Timely conferences will be held to understand the patient as a whole and to assist in finding appropriate problem-solving strategies.
３．実習最終日には学内で, 病棟実習で受け持った患者への看護プロセスを振り返り，成人慢性期看護につい
て考えを深める。
A review meeting will be held on campus on the last day of the practice training.
成績評価の方法及び基準 / Record and Evaluation method
実習スケジュール全般として実習内容，実習態度，課題レポ－ト，平常点を含み，総合的に判断して評価する。
学内実習：担当教員(100％)が評価する。
病棟実習：担当教員(50％)および病棟指導者（50％）,計100％で評価する。
Comprehensive evaluation based on learning attitude.
・On-campus training: Evaluated by the teachers (100%).
・Hospital training: Teachers in charge (50%) and hospital mentor (50%), evaluated by a total of 100%
授業時間外学習 / Self-study
臨地実習の目的・目標を常に意識し，事前に実習に関連する基礎知識を学習して実習に臨むこと。
また，毎日の実習後には，経験した実習内容について参考書等を活用して復習を行い，事象に対する
理解を深めること。
Students are required to prepare for each practice training in advance.
